Your Body Has Its Own Birthday

What glucose, inflammation, muscle, and fitness reveal about how well you’re aging
Two people walk into a doctor’s office on the same day. Both are 52. Both eat reasonably well. Both exercise a few times a week.
One has stable blood sugar, low inflammation, strong muscle mass, and excellent cardiovascular fitness for their age. The other has rising fasting glucose, signs of chronic low-grade inflammation, declining lean tissue, and cardiovascular fitness that’s been sliding for a decade.
Same birthday. Same age on paper.
But their risk profiles—and the way their bodies are aging—may look remarkably different.
That difference isn’t determined by genetics alone. Many of the systems that influence how we age are measurable and, importantly, modifiable.
What “Biological Age” Actually Means
Your chronological age is the number on your driver’s license.
“Biological age” is a broader concept used to describe how aging is showing up in your body’s physiology. Researchers use several different models to study it, and there isn’t one universally accepted test that can tell you exactly how old your body is.
But we can measure important indicators associated with healthspan, disease risk, physical function, and longevity.
And that’s where this gets useful.
Four areas that tell us a great deal about healthy aging are glucose regulation, inflammation, muscle and strength, and cardiovascular fitness.
Each one is measurable.
Each one can be tracked over time.
And each one gives us something we can potentially act on.
Glucose Control: Your Metabolic Health
Your body’s ability to regulate blood sugar is an important part of healthy aging.
Insulin resistance can develop gradually, sometimes before fasting glucose or HbA1c reaches the threshold for prediabetes or diabetes.
That’s why, in the right clinical context, looking beyond a single fasting glucose can provide additional information about metabolic health.
Markers that may be useful include:
→ Fasting glucose — your blood sugar after an overnight fast
→ HbA1c — an estimate of average blood glucose over the previous few months
→ Fasting insulin — which can provide additional metabolic information in certain patients
→ HOMA-IR — a calculation using fasting glucose and insulin that is commonly used as an estimate of insulin resistance
→ Post-meal glucose patterns — which can sometimes provide insight into how your body responds to food
Not everyone needs every marker.
The goal is to identify metabolic changes early enough to do something meaningful about them.
Nutrition, physical activity, sleep, weight management when appropriate, and sometimes medication can all significantly improve metabolic health.
Inflammation: The Background Signal
Inflammation isn’t inherently bad. It’s part of a healthy immune response.
The concern is persistent, low-grade systemic inflammation, which is associated with cardiovascular disease, metabolic dysfunction, and many age-related conditions.
And unlike the inflammation you experience with an injury or infection, low-grade inflammation often doesn’t produce obvious symptoms.
One commonly used marker is:
→ hs-CRP (high-sensitivity C-reactive protein) — a blood marker that can provide information about systemic inflammation and cardiovascular risk
Depending on someone’s health history and risk factors, other metabolic and cardiovascular biomarkers may also provide useful context.
But inflammation isn’t something we interpret from one number alone.
Poor sleep, smoking, excess visceral fat, metabolic dysfunction, chronic disease, infections, and other factors can all influence inflammatory markers.
The goal isn’t simply to “get inflammation down.”
It’s to understand why it’s elevated in the first place.
Muscle: Your Metabolic Reserve
Muscle isn’t just a fitness metric.
Skeletal muscle plays an important role in glucose metabolism, physical function, mobility, balance, and our ability to remain independent as we age.
Beginning as early as our 30s and 40s, muscle mass gradually declines with age, and that loss can accelerate later in life.
But age-related muscle loss isn’t something we simply have to accept.
We can measure and track muscle and physical function using tools such as:
→ DEXA scanning — which can provide information about lean mass, fat mass, and bone density
→ Grip strength — a simple measure of physical function that has been strongly associated with health outcomes
→ Strength and functional movement assessments — which help establish a baseline and track progress over time
Preserving muscle generally requires two things many adults aren’t getting enough of:
Adequate protein and progressive resistance training.
Sleep, overall nutrition, activity level, medications, chronic disease, and certain hormonal or medical conditions can also influence muscle mass and recovery.
When symptoms or history suggest a medical contributor—such as clinically significant thyroid or testosterone abnormalities—those deserve appropriate evaluation too.
The goal isn’t simply to weigh less as you age.
It’s to remain strong.
Cardiovascular Fitness: One of Our Strongest Health Indicators
Cardiorespiratory fitness reflects your body’s ability to take in, transport, and use oxygen during physical activity.
One of the best-known measures is VO₂ max.
And this is one of the most compelling areas in longevity research.
Higher cardiorespiratory fitness is consistently associated with lower cardiovascular and all-cause mortality, while low fitness is associated with substantially higher long-term health risk.
In other words, fitness isn’t just about whether you can run a mile.
It’s an important marker of health.
Ways we can assess cardiovascular fitness include:
→ VO₂ max testing or validated estimates
→ Resting heart rate trends over time
→ Heart rate recovery after exertion
→ Functional exercise capacity
And unlike many risk factors we can’t control, cardiovascular fitness is highly trainable.
Moderate-intensity aerobic exercise helps build an aerobic base. Higher-intensity training, when appropriate for the individual, can improve cardiorespiratory capacity further.
You don’t have to become an endurance athlete.
You simply have to give your cardiovascular system a reason to adapt.
The Number That Matters Most Isn’t on Your License
Your birthday tells us how long you’ve been alive.
It doesn’t tell us everything about how well you’re aging.
That’s where objective health markers become useful.
Glucose regulation. Inflammation. Muscle and strength. Cardiovascular fitness.
These aren’t abstract wellness concepts. They’re measurable indicators that can help us identify risk, establish a baseline, and track whether the things you’re doing are actually moving your health in the right direction.
And these systems don’t exist independently.
More muscle can improve glucose disposal.
Physical activity can improve insulin sensitivity and cardiovascular fitness.
Improving metabolic health may positively affect inflammatory risk.
Better sleep and nutrition support nearly all of them.
The benefits can compound.
And that’s what makes this approach to healthy aging so powerful.
